Enrollment

At Christ the Vine Lutheran School we believe that each child is a unique creation of God, who arrives with a natural curiosity about and an innate sense of the presence of God.  Our desire is to nurture and encourage that sense of wonder, and communicate the love of God.

 

While it is our desire to provide a Christ centered education to all, should you have concerns or know that your child has a special learning need, communication disorder, behavioral challenge or health issue, please discuss that with us prior to admission.

 

Our school is a mission of Christ the Vine Lutheran Church.  If tuition is a hardship for your family, please contact Seanna Bloemer at sbloemer.ctv@gmail.com for information regarding tuition assistance.

 

To enroll your child, click on the appropriate link below to access the enrollment form. Please download and save the form to your computer before entering any data. If you choose to type in the information at your computer, it is suggested that you use “overtype mode” This will replace the lines as you type, rather than pushing them to the right. If pressing the Ins/Insert key doesn’t work, it is easy to enable overtype mode in Word. Look here for instructions as to how.

When finished, print it off, sign at the bottom of page 2, and have your child complete page 4. You may wish to make a copy for your records.

Delivery options:

1 -Scan the finished form, save it as a pdf, and email it as an attached file to sbloemer.ctv@gmail.com, copied to kristina@christthevinelutheran.org.

2 -Alternatively you may send the finished form by postal mail to the attention of Seanna Bloemer at the church address as shown at the top of the form.

3 -Or if you prefer, drop the finished form off at the church office during office hours (8:00-12:30 M-F).
